The form of the register of insured persons has been updated, for which additional insurance premiums for a funded pension, the procedure for its provision and the formats for their provision in in electronic format.

The register is submitted by the employer, listing additional insurance contributions to the funded pension of insured persons.

The register is formed separately in relation to the amounts of additional insurance contributions to the funded pension of each insured person (if they are paid).

Clarified presentation order employer of the register of insured persons on hard copy and in electronic form with enhanced qualified electronic signature.

The update affected the registry form for additional insurance premiums DSV-3. It must be submitted by all organizations listing additional. insurance contributions for employees to the funded part of the pension.

The register is submitted quarterly no later than 20 days after the end of the quarter.

The Register specifies:

The total amount of transferred funds,

Name of the insured person,

The amount of additional contributions transferred,

Employer contributions.

The register is submitted to the FIU no later than 20 days from the date of the end of the quarter during which additional contributions were transferred and contributions were paid.

The procedure for submitting a register of insured persons for whom additional insurance premiums for a funded pension are listed and employer contributions are paid, in accordance with Appendix No. 2;

The format for submitting the register of insured persons for whom additional insurance contributions to the funded pension are listed and employer contributions are paid, in electronic form (data format), in accordance with Appendix No. 3.

2. Recognize as invalid:

ORDER
SUBMISSION OF THE REGISTER OF INSURED PERSONS,
FOR WHICH ADDITIONAL INSURANCE CONTRIBUTIONS ARE PLACED
FOR THE CONDUCTIVE PENSION AND EMPLOYER CONTRIBUTIONS PAID

I. General provisions

1.1. The procedure for submitting the register of insured persons for whom additional insurance contributions to the funded pension are transferred and employer contributions paid (hereinafter referred to as the register of insured persons) has been prepared on the basis of the provisions of parts 4, 6, 7 of Article 9 of the Federal Law of April 30, 2008 N 56- Federal Law "On additional insurance contributions for funded pensions and state support for the formation of pension savings" (hereinafter - Federal Law of April 30, 2008 N 56-FZ).

1.2. The register of insured persons is submitted by the employer, listing:

additional insurance premiums for the funded pension of insured persons;

employer's contributions in favor of the insured persons (if any).

1.3. In accordance with Part 4 of Article 9 of the Federal Law of April 30, 2008 N 56-FZ, the employer, simultaneously with the transfer of additional insurance premiums, forms a register of insured persons, which contains the following information:

1) the total amount of transferred funds, including:

a) the amount of all additional insurance contributions transferred to the funded pension of the insured persons;

b) the amount of all paid employer contributions (if any);

2) the number of the payment order and the date of its execution;

3) the insurance number of the individual personal account of each insured person;

4) last name, first name, patronymic (last - if any) of each insured person;

5) the amount of additional insurance contributions transferred to the funded pension of each insured person;

6) the amount of the employer's contributions paid in favor of each insured person (if any).

The registers are formed separately for the amounts of additional insurance contributions to the funded pension and for the amounts of the employer's contributions (if they are paid).

1.4. In accordance with Parts 6 and 7 of Article 9 of the Federal Law of April 30, 2008 N 56-FZ, the register of insured persons is submitted by the employer to the territorial body of the Pension Fund of the Russian Federation (hereinafter referred to as the territorial body of the PFR) or through a multifunctional center for providing state and municipal services(hereinafter referred to as the MFC) no later than 20 days from the end of the quarter during which additional insurance premiums for the funded pension were transferred and employer contributions were paid (if any).

employers, average headcount whose employees for the previous calendar year exceed the number established by part 7 of article 9 of the Federal Law of April 30, 2008 N 56-FZ, as well as newly created (including through reorganization) organizations, the number of employees of which exceeds the number of employees established by part 7 of article 9 Federal Law of April 30, 2008 N 56-FZ number, represent the register of insured persons in the form electronic document, signed with an enhanced qualified electronic signature in accordance with the Federal Law of April 6, 2011 N 63-FZ "On Electronic Signature" (Sobranie Zakonodatelstva Rossiyskoy Federatsii, 2011, N 15, Art. 2036; N 27, Art. 3880; 2012, No. 29, article 3988; 2013, No. 14, article 1668; No. 27, article 3463; article 3477; 2014, No. 11, article 1098; No. 26, article 3390; 2016, No. 1, article 65; N 26, item 3889).

In the same manner, the register of insured persons can be submitted to the territorial body of the PFR or through the MFC by employers whose average number of employees for the previous calendar year is the number established by Part 7 of Article 9 of the Federal Law of April 30, 2008 N 56-FZ or less.

II. Submission by the employer of the register of insured persons
on paper

2.1. Employers whose average number of employees for the previous calendar year does not exceed the number established by Part 7 of Article 9 of the Federal Law of April 30, 2008 N 56-FZ may submit a register of insured persons on paper.

2.2. The employer submits two copies of the register of insured persons on paper.

2.3. If the volume of the register is two or more sheets, the sheets are numbered and stitched together. The brochure is stitched with thread. The ends of the fastening thread are brought out from the back side of the register of insured persons, tied and sealed with a sheet of paper, on which the inscription is made: "The register is stitched, numbered and sealed ... sheets.". This inscription is signed by the head or chief accountant of the employer (if there is a power of attorney for this from the head of the organization) and certified by the seal of the organization (if any).

III. Submission by the employer of the register of insured persons
persons in electronic form with enhanced qualified
electronic signature

3.1. Employers whose average number of employees for the previous calendar year exceeds the number established by Part 7 of Article 9 of the Federal Law of April 30, 2008 N 56-FZ, as well as newly created (including through reorganization) organizations whose number of employees exceeds the established part 7 of article 9 of the Federal Law of April 30, 2008 N 56-FZ number, represent the register of insured persons in the form of an electronic document signed with an enhanced qualified electronic signature.

In the same manner, the register of insured persons may be submitted by employers whose average number of employees for the previous calendar year is the number established by Part 7 of Article 9 of the Federal Law of April 30, 2008 N 56-FZ or less.

3.2. In the cases specified in paragraph 3.1, the employer generates a register in the form of a file in accordance with the approved Format for presenting the register of insured persons for whom additional insurance premiums for a funded pension are listed and employer contributions are paid, in electronic form (data format).

3.3. The file with the register of insured persons is signed with an enhanced qualified electronic signature of the head or chief accountant of the employer (if there is a power of attorney for this from the head of the organization).

3.4. The submission of the register of insured persons in electronic form with an enhanced qualified electronic signature is carried out in a closed (encrypted) form using certified cryptographic information protection tools used in the territorial bodies of the PFR.

3.5. Worker territorial authority The PFR, having received the file with the register of insured persons in electronic form, decrypts it, checks the validity of the enhanced qualified electronic signature of the employer, checks the amounts of additional insurance premiums for funded pension and employer contributions (if any) indicated in the register of insured persons with the data recorded in program complex of the Pension Fund of the Russian Federation, checks the data in the register of insured persons for compliance with the details of the employer, insurance numbers of individual personal accounts (SNILS) with the last names, first names and patronymics (the latter, if any) of the insured persons for whom additional insurance premiums for the funded pension are transferred and (or) the employer's contributions are paid (if they are paid), and in the absence of errors, signs the file with its enhanced qualified electronic signature, encrypts and transmits (sends via secure telecommunication channels) to the employer.

3.6. If errors are found, the register of insured persons together with the error protocol issued by software package The Pension Fund of the Russian Federation as a result of checking the register is returned to the employer for correction and re-submission of the register of insured persons.

On the increase in the authorized capital of the State Unitary Enterprise "Moscow Metro"

In accordance with the Budget Code of the Russian Federation and Federal Law No. 161-FZ of November 14, 2002 "On State and Municipal unitary enterprises": 1. To increase in 2011 the authorized capital of the State Unitary Enterprise of the city of Moscow "Moscow Order of Lenin and the Order of the Red Banner of Labor Metro named after V.I. Lenin" (GUP "Moscow Metro") by 11,200,000,000 rubles including: - RUB 8,500,000,000 for the purpose of investing in capital construction facilities of the Moscow Metro in accordance with the Appendix to this Order; - RUB 2,700,000,000 for purposes according to the feasibility study for increasing statutory fund State Unitary Enterprise "Moscow Metro" (hereinafter - Feasibility Study). 2. Establish that in order to increase the authorized capital of the State Unitary Enterprise "Moscow Metro" (item 1), the following conditions must be met: 2.1. Submission within a week from the date of entry into force of this order, together with the agreed feasibility study in the amount of 11,200,000,000 rubles (clause 1), indicating the purposes, volumes and periods of spending the funds provided from the budget of the city of Moscow. 2.2. Conducted by the Department economic policy and development of the city of Moscow, the examination of the feasibility study (clause 2.1) within two weeks from the date of its submission. 3. Establish that measures to increase the authorized capital of the State Unitary Enterprise "Moscow Metro" are carried out by the Department of Property of the City of Moscow, subject to a positive conclusion on the feasibility study (clause 2.2). 4. Establish that the transactions specified in the feasibility study (clause 2.1) related to investment in the capital construction of metro facilities are approved by the owner of the property in accordance with the provisions of Article 23 of the Federal Law of November 14, 2002 N 161-FZ "On State and Municipal Unitary Enterprises". 5. Together with the Department of Transport and Road Infrastructure Development of the City of Moscow, ensure quarterly submission to the Department of Property of the City of Moscow and reports of the State Unitary Enterprise "Moscow Metro" on the implementation of the feasibility study, indicating the purposes of spending the provided budget funds, volumes of their spending and achievement of planned indicators of the feasibility study. 6. Information about completed vaccinations only. For example, if a vaccination involves four vaccinations, then only those who were vaccinated four times would be included in the report;

Information about all revaccinations;

Information about vaccinations carried out with vaccines (anatoxins) of domestic or foreign production, registered and authorized for use in the prescribed manner;

Information about vaccinations carried out within the framework of the national calendar of preventive vaccinations, the calendar of preventive vaccinations according to epidemic indications and additional immunization within the framework of the priority national project;

2.3. line 01 "vaccination against whooping cough" includes the number of children who received three doses of pertussis, diphtheria and tetanus vaccine;

2.4. line 02 "pertussis revaccination" shall include the number of children who received the 4th pertussis, diphtheria and tetanus vaccine at the age of 18 months or 1 year after vaccination was completed;

2.5. line 03 "diphtheria vaccination - total" shall include the number of persons of all age groups who were vaccinated three times with pertussis, diphtheria, and tetanus vaccine or twice vaccinated with diphtheria-tetanus toxoid, antigen-reduced diphtheria-tetanus toxoid, and diphtheria toxoid, as well as the number of children vaccinated twice with whooping cough, diphtheria, and tetanus vaccine, if after the last vaccination passed more than 12 months;

2.6. line 04 "including children" shall include the number of children under the age of 17 years inclusive (17 years 11 months 29 days) vaccinated three times with pertussis, diphtheria and tetanus vaccine or twice vaccinated with diphtheria-tetanus toxoid, diphtheria-tetanus toxoid with reduced the content of antigens and diphtheria toxoid, as well as the number of children vaccinated twice with pertussis, diphtheria and tetanus vaccine, if more than 12 months have passed since the last vaccination;

2.7. line 05 "diphtheria revaccination - total" includes the number of children who received diphtheria revaccination with pertussis, diphtheria and tetanus vaccine, diphtheria-tetanus toxoid or antigen-reduced diphtheria-tetanus toxoid, as well as the number of persons aged 18 years and over older, who received revaccination against diphtheria with antigen-reduced diphtheria-tetanus toxoid every 10 years;

2.8. line 06 "including children" shall include the number of children up to and including 17 years of age (17 years 11 months 29 days) who received revaccination against diphtheria with pertussis, diphtheria and tetanus vaccine, diphtheria-tetanus toxoid or antigen-reduced diphtheria-tetanus toxoid ;

2.9. line 07 "vaccination against tetanus - total" shall include the number of persons vaccinated three times with pertussis, diphtheria and tetanus vaccine, twice vaccinated with pertussis, diphtheria and tetanus vaccine if more than 12 months have passed since the last vaccination, and also vaccinated twice with diphtheria-tetanus toxoid, twice vaccinated with diphtheria-tetanus toxoid with a reduced content of antigens and tetanus toxoid. This line also includes the number of persons vaccinated with a double dose (1.0 ml) of tetanus toxoid;

2.10. line 08 "including children" shall include the number of children under 17 years of age inclusive (17 years 11 months 29 days) vaccinated three times with the pertussis, diphtheria and tetanus vaccine, twice vaccinated with the pertussis, diphtheria and tetanus vaccine, if the last vaccination has passed more than 12 months, as well as twice vaccinated with diphtheria-tetanus toxoid, diphtheria-tetanus toxoid with a reduced content of antigens, tetanus toxoid or single vaccinated with a double dose (1 ml) of tetanus toxoid;

2.11. line 09 "tetanus revaccination - total" includes the number of persons who received revaccination with pertussis, diphtheria and tetanus vaccine, diphtheria-tetanus toxoid, antigen-reduced diphtheria-tetanus toxoid, tetanus toxoid, as well as those previously vaccinated who received tetanus toxoid ( 0.5 ml) for emergency tetanus prophylaxis;

2.12. line 10 "including children" shall include the number of children up to and including 17 years of age (17 years 11 months 29 days) who received revaccination with pertussis, diphtheria and tetanus vaccine, diphtheria-tetanus toxoid, diphtheria-tetanus toxoid with a reduced content of antigens, tetanus toxoid, as well as those who received tetanus toxoid (0.5 ml) during emergency prophylaxis;

2.13. Row 11 "Vaccination against poliomyelitis" shall include the number of children who received three doses of polio vaccination;

2.14. line 12 "revaccination against polio" includes the number of children who received the 4th, 5th, 6th vaccination against polio;

2.15. line 13 "vaccination against measles - total" includes the number of persons vaccinated against measles once;

2.16. line 14 "including children" includes the number of children up to and including 17 years of age (17 years 11 months 29 days) who received their first measles vaccine;

2.17. line 15 "revaccinated against measles" includes the number of persons who received a second measles vaccination;

2.18. line 16 "including children" includes the number of children up to and including 17 years of age (17 years 11 months 29 days) who received a second measles vaccine;

2.19. line 17 "vaccination against mumps" includes the number of children from 12 months of age who were vaccinated once against mumps;

2.20. line 18 "mumps revaccination" includes the number of children who received a second mumps vaccination;

2.21. line 19 "rubella vaccination" includes the number of children from 12 months of age vaccinated against rubella once;

2.22. line 20 "rubella revaccination" includes the number of persons who received a second rubella vaccination;

2.23. line 21 "inoculations against typhoid" shall include the number of persons vaccinated against typhoid;

2.24. line 22 "vaccinations against tuberculosis - total" shall include the total number of children vaccinated against tuberculosis;
