Section "Qualification characteristics of positions of workers in the field of healthcare" of the "Unified qualification handbook positions of managers, specialists and employees ”(hereinafter referred to as CEN), approved Order of the Ministry of Health and Social Development of the Russian Federation dated July 23, 2010 No. 541n(hereinafter referred to as Order No. 541n), qualification characteristics have been established for certain positions of specialists with higher medical education. Previously, the qualification characteristics approved by Order No. 869 of November 6, 2009. Many general provisions in the new legal act remained unchanged.

The CEN and the qualification characteristics themselves should contribute to:

• Proper selection and placement of personnel in the structure of medical personnel;
• medical personnel;
• Division of labor between managers, specialists;
• Ensuring the unity of approaches in determining these categories of medical personnel and requirements for them.

Besides, qualification characteristics applied as normative documents and also serve as the basis for the development job descriptions medical personnel, containing a specific list of job responsibilities of medical personnel, taking into account the characteristics of work.

Order No. 541n provides for qualification characteristics for:

• Positions of managers;
• Positions of specialists (with higher medical, higher professional and secondary medical education);
• Positions of junior medical personnel;
• Positions of other personnel of medical and labor workshops at medical organizations.

The titles of positions of specialists with higher medical education are established in accordance with the Qualification requirements for them, approved by the new Order of the Ministry of Health of Russia dated 08.10.2015 No. 707n, which entered into force on November 7, 2015 (previously Order of the Ministry of Health and Social Development of Russia dated July 7, 2009 No. 415n). For example, the title of the position "doctor-therapist" corresponds to the medical specialty "therapy".

The names of the positions of medical specialists are established in accordance with the names of medical specialties, the nomenclature of which is approved in the prescribed manner ( Order of the Ministry of Health of Russia dated October 7, 2015 No. 700n(previously operated Order of the Ministry of Health and Social Development of the Russian Federation of April 23, 2009 No. 210n, which lost its legal force on November 27, 2015), Order of the Ministry of Health and Social Development of the Russian Federation dated April 16, 2008 No. 176n).

General qualification characteristic - "Doctor-specialist"

It should be borne in mind that Order No. 541n establishes a general qualification characteristic for the position of "Specialist Doctor". It applies to those positions of medical specialists for which this section of the CSA does not provide for separate qualification characteristics.

For specialists with higher professional (non-medical) education accepted for this position before October 1, 1999, the title of the position "Doctor-laboratory assistant" is retained.

A specialist with a medical education may receive the official title "Senior" if he manages his subordinate performers.

Order No. 541n also stipulates the situation when a specialist doctor is appointed to the position of a trainee doctor. In doing so, he:

• Has a break in work in the specialty for more than 5 years, or
• Changes the specialty profile.

Job Responsibilities

"Job Responsibilities" is a list of the main functions of medical personnel that can be entrusted to an employee holding this position. Technological homogeneity and interconnectedness of work, as well as professional education received, are taken into account.

Must know

The “Must Know” section contains the basic requirements for an employee with regard to special knowledge, as well as knowledge of legislative and other, regulations, instructions and other documents, methods and means that the employee must be able to apply in the performance of official duties.

Requirements for the qualification of medical personnel

In chapter " Qualification Requirements» levels of required vocational education the employee necessary to perform the duties assigned to him, as well as the required length of service.

At the same time, persons who do not meet the established qualification requirements, but have sufficient practical experience and those who perform qualitatively and in full the duties assigned to them, on the recommendation of the attestation commission of a medical organization, as an exception, can be appointed to the appropriate positions. The employer takes measures to further the mentioned persons (clause 6 of Order No. 541n).

1. The section "Qualification characteristics of positions of workers in the field of healthcare" of the Unified Qualification Directory of the positions of managers, specialists and employees (hereinafter referred to as the CSA) is intended to promote the correct selection and placement of personnel, improve their business skills, division of labor between managers, specialists, and also ensure unity approaches in determining the job responsibilities of these categories of workers and the qualification requirements for them and consists of five sections: I - " General provisions"; II - "Positions of managers"; III - "Positions of specialists"; IV - "Positions of junior medical personnel"; V - "Positions of other personnel of medical and labor workshops at medical organizations".

2. The qualification characteristics contained in the section "Qualification characteristics of positions of workers in the field of healthcare" of the CSA (hereinafter referred to as qualification characteristics) are used as regulatory documents, and also serve as the basis for the development of job descriptions containing a specific list of job responsibilities, taking into account the characteristics of the work of employees medical organizations. The positions of specialists with higher medical and pharmaceutical education are established in accordance with the Qualification requirements for specialists with higher and postgraduate medical and pharmaceutical education in the field of healthcare, approved in the prescribed manner by the Ministry of Health and Social Development of Russia. For example, the title of the position "therapist" corresponds to the medical specialty "therapy".

The titles of the positions of medical specialists and pharmacist-specialists are established in accordance with the names of medical and pharmacist specialties, the nomenclature of which is approved in the prescribed manner.

The title of the post "doctor-laboratory assistant" is retained for specialists with higher professional (non-medical) education hired to this position before October 1, 1999.

In the event that a specialist doctor (pharmacist) has a break in his specialty for more than 5 years or changes the profile of a specialty that requires special training and qualifications, he is appointed to the position of a trainee doctor (trainee pharmacist) for the period of professional retraining in the prescribed manner in order to obtain special knowledge, skills and abilities necessary for independent work in the position of a specialist doctor (pharmacist), and obtaining a certificate in the relevant specialty.

During this period of time, he can perform the duties stipulated by the qualification characteristics of a specialist doctor (doctor - specialist of the State Sanitary and Epidemiological Service, pharmacist), only under the guidance of a specialist doctor (doctor - specialist of the State Sanitary and Epidemiological Service, pharmacist), who has a certificate of a specialist in the relevant specialty, answering on par with him for his actions.

3. The names of the positions of deputy heads of a medical organization (chief physician, director, head, head) are supplemented with the name of the section of work, which he supervises. For example, "deputy chief physician for medical affairs", "deputy chief physician for work with nursing personnel", etc.

The titles of the positions of the heads of structural subdivisions (departments, departments, laboratories, offices, detachments, etc.) are supplemented with the name of the medical position corresponding to the specialty according to the profile structural unit. For example, "the head of the surgical department is a surgeon."

4. The qualification description of each position contains three sections: "Job Responsibilities", "Must Know" and "Qualification Requirements".

The section "Responsibilities" establishes a list of the main functions that can be entrusted to an employee holding this position, taking into account the technological homogeneity and interconnectedness of the work received professional education.

The "Must Know" section contains the basic requirements for an employee with regard to special knowledge, as well as knowledge of legislative and other regulatory legal acts, regulations, instructions and other documents, methods and means that the employee must be able to apply in the performance of official duties.

The "Qualification Requirements" section defines the levels of required professional education of an employee necessary to perform the duties assigned to him, as well as the required length of service. The levels of required vocational training specified in the "Qualification Requirements" section are given in accordance with the Law Russian Federation"On Education" dated July 10, 1992 N 3266-1 (Sobraniye Zakonodatelstva Rossiyskoy Federatsii, 1996, N 3, Art. 150) and other normative legal acts.

5. The official title "senior" is established on the condition that the specialist manages the executors subordinate to him.

6. Persons who do not have the appropriate additional professional education or work experience established by the qualification requirements, but who have sufficient practical experience and perform their official duties qualitatively and in full, on the recommendation of the attestation commission of a medical organization, as an exception, may be appointed for relevant positions as well as persons with special training and the necessary length of service.

7. In medical organizations, the names of positions: "midwife", "nurse", "nurse", "packer", replaced by males, are named respectively: "obstetrician", "nurse brother (nurse)", "orderly", " packer".

<*>The title of the position "Chief Physician" can be used only if the head of the medical organization has a higher professional (medical) education.

Job responsibilities. Manages the activities of a medical organization. Organizes the work of the organization's team to provide and provide quality medical services to the population. Provides organization of medical and preventive, administrative and economic financial activities organizations. Carries out an analysis of the organization's activities and, based on an assessment of its performance indicators, takes necessary measures to improve the forms and methods of work. Approves staffing, financial plan, annual report and annual balance sheet of the organization. Ensures fulfillment of obligations under the collective agreement. Provides employees with timely and full payment of wages. Represents the organization in state, judicial, insurance and arbitration bodies, at international events, in state and public organizations on issues in the field of healthcare, previously agreed with the higher authority in charge of this organization. Interacts with local governments, civil defense services, disaster medicine, territorial bodies internal affairs and other operational services. Takes part in conferences, seminars, exhibitions. Provides preventive measures to prevent industrial injuries and occupational diseases.

Job responsibilities. Carries out the issue of sanitary and hygiene items, patient care, other medical products, medicinal herbs. Provides storage and control over the shelf life of medicinal herbs and medical devices. Participates in the acceptance, placement and packaging of goods, using necessary equipment and complying with health and safety regulations. Conducts health education and information work on the use of medical devices.

Must know: the basics of the organization of the pharmaceutical service; basic regulatory legal acts on pharmacy issues; nomenclature of medicinal herbs and medical products; fundamentals of labor legislation; internal labor regulations; rules on labor protection and fire safety.

Qualification requirements. Secondary vocational education in the specialty "Pharmacy" without presenting requirements for work experience.

Job responsibilities. Supervises the work of nurses and cleaners to keep the premises of a medical organization (unit) clean and tidy, provides the serviced unit with household equipment, overalls, hygiene items, stationery, detergents, bed linen and underwear for patients. Makes a change of bathrobes, towels for employees of a medical organization. Draws up requests for the repair of premises, equipment, inventory and supervises its implementation. Provides power supply units (buffet, canteen) with equipment, utensils and monitors their correct labeling and use. Maintains accounting records.

Must know: the expiration dates of linen and equipment used in a medical organization (department); methods of sanitizing inventory; conditions of operation and storage of inventory; forms of accounting and reporting documentation and rules for filling them out; rules for compliance with the sanitary and hygienic regime in a medical organization (subdivision); internal labor regulations; rules on labor protection and fire safety.

Job responsibilities. Performs packing and dosing work medicines, medical devices, using the necessary equipment, means of mechanization, observing the rules for their operation. Participates in the acceptance and distribution of goods between departments of the pharmacy.

Must know: basic methodological and regulatory documents on pharmacy issues; internal labor regulations; rules on labor protection and fire safety.

Qualification requirements. Secondary (complete) general education and additional training in the direction of professional activity without presenting requirements for work experience.

Job responsibilities. Cleans rooms in a medical facility. Helps the senior nurse in obtaining medicines, tools, equipment and delivering them to the department. Receives from the hostess and ensures the proper storage and use of linen, household equipment, dishes and detergents. Removes bedside tables from bedridden patients after each meal. At the direction of the ward nurse, she accompanies patients to the treatment and diagnostic rooms. Performs the functions of a courier, carries out washing of pharmacy dishes. Informs the hostess about malfunctions in the heating system, water supply, sewerage and electrical appliances. Prepares rooms and bathrooms. Systematically (after each patient) carries out sanitary and hygienic treatment of the bath and washcloths. Provides assistance to patients when taking a hygienic bath, when undressing and dressing. In the absence of a junior nurse to care for the sick, she receives underwear and bed linen from the hostess and changes them. Receives prepared food at the catering department, checks it by weight and count. Signed in the distribution sheet. Produces food heating. Qualification requirements. Secondary (complete) general education without presenting requirements for work experience.

Job responsibilities. Performs, together with the paramedic, carrying, loading and unloading patients and victims during their transportation, assists the doctor and paramedic in performing diagnostic and therapeutic procedures, transfers medical equipment. Provides assistance to medical personnel accompanied by patients. Drives an ambulance classified as one of the categories "B" or "C". Refuels the vehicle with fuel, lubricants and coolant. Prepares travel documents. Checks technical condition and receiving the car before leaving the line. Delivery of the car and putting it in the allotted place upon returning from work. Eliminates minor operational malfunctions of the rolling stock that have arisen during work on the line and do not require disassembly of the mechanisms. Carries out sanitary and hygienic treatment of the car interior after each patient (injured). Reports vehicle problems.

Job responsibilities. Teaches unskilled workers, students, modern methods of performing production operations, working on machines, machine tools, etc. Plans educational work and keeps records of the implementation of industrial training programs, the results of the work of trainees and their progress. Supervises the provision of safe working conditions for trainees, the equipment of their workplaces with everything necessary for the performance of work. 5. The titles of the positions of heads (heads) of structural subdivisions (departments, departments, laboratories, offices, detachments, etc.) are supplemented by the title of the position of a doctor corresponding to the profile of the structural subdivision. For example, "the head of the surgical department is a surgeon."

6. In a medical organization providing specialized medical care, or if there is a structural unit in a medical organization providing specialized medical care, the title of the position "doctor of the admission department" is supplemented with the title of the position of a doctor of the relevant specialty. For example, "the doctor of the emergency department - the doctor of emergency medical care."

7. The names of the positions "obstetrician", "nurseman", "packer" filled by females are named respectively: "midwife", "nurse", "packer"; and the name of the position "nurse", filled by males, is called - "medical brother (nurse)".
